Activate Instructions:
2026 - 2027 School Year
Read and follow the instructions. Student/athletes are not eligible until they are checked off online through https://www.registermyathlete.com/login/
- Registration for Parents/Guardians CREATE A PARENT/GUARDIAN ACCOUNT:
- Go to https://www.registermyathlete.com/login/
- Click Create an Account * You only need ONE account, even if you have children in more than one high school and/or junior high * Do not create another account if you have used Register My Athlete/Aktivate in the past
- Fill in your personal account information. You will be using the site as a Parent/Guardian.
- Click Create Account
- Shortly after creating your account, you will receive an email with a 6-digit Verification Code. Use this code to verify your account when prompted. * Do not close your current tab. You will need to open your email in another tab and find the verification email in your email inbox (it may take a few minutes to appear, so be patient). REGISTER YOUR STUDENT FOR AN ACTIVITY:
- Login with your email address and password
- Under the Parents header, click the button labeled Click here to start/complete athlete registrations
- Click Start/Complete Registrations under "What would you like to do?"
- Click the New Registration + button in the upper right hand corner
- Start by clicking the red Select School bar and follow the directions as they appear
- Continue following the steps presented in the red bars

Create a Username and password. You only need to register once. If you may be participating in more than 1 sport, then please add all of them when registering. Football, please choose both the 11 man football and Spring Football, unless you are senior.
All forms must be 100% complete before trying out or participating in athletics at Monarch. All paperwork must be completed prior to going to practice, tryouts, or off-season workouts.
Checklist for requirements for clearance:
- Current Physical, completed on the FHSAA Physical Form:
- FHSAA forms available online on the Aktivate registration site.
- Please include Page 4 only, unless a followup is required and then also include page 5 of the physical.
- Physical must be stamped by the doctor's office.
- DO NOT SUBMIT YOUR IMMUNIZATION RECORDS.

- Current proof of insurance (copy of card) and it is required every year. Must have student's name on it or other proof that the student athlete is covered for the current school year.
NOTE: Get the 24 hour coverage because athletics is an after school activity.
NOTE: Football players, if getting school insurance, be sure to get the coverage for football. Also, this does not cover Spring football or Summer football activities, more school insurance must be purchased.
- Birth Certificate Copy.
- FHSAA Concussion Certificate (watch video, save certificate as pdf file and upload to the Athletic Clearance site) Must have student's first and last name on the certificate. Link available on activate.com site.
- FHSAA Heat Illness Certificate(watch video, save certificate as pdf file and upload to the Athletic Clearance site) Must have student's first and last name on the certificate. Link available on activate.com site.
- FHSAA Sudden Cardiac Arrest Certificate (watch video, save certificate as pdf file and upload to the Athletic Clearance site) Must have student's first and last name on the certificate. Link available on activate.com site.
- FHSAA Anti Bullying and Hazing Certificate (watch video, save certificate as pdf file and upload to the Athletic Clearance site) Must have student's first and last name on the certificate. Link available on activate.com site.
- FHSAA Sportsmanship Certificate (watch video, save certificate as pdf file and upload to the Athletic Clearance site) Must have student's first and last name on the certificate. Link available on activate.com site.
- Parents/Guardians and the student must complete the online registration/signatures forms
- Must have at least a 2.0 unweighted GPA to be eligible for participation. Beginning of the school year (except incoming 9th graders) and at the beginning of the 2nd semester. If you have completed any online or summer school courses, be sure to let your school counselor know immediately. You will have until the beginning of each semester term to be eligible.
- If student/athlete has come from a different school, other than middle school, a GA4 form must be completed by the parent/guardian FHSAA.Com site with the form: https://fhsaa.com/documents/2023/7/20/GA04_affidavit_07_2023_.pdf Give the signed form to Mr. Hester.
